To provide proper oversight for the Annual David Manlove Memorial Tournament, we are asking for each team’s assistance by Thursday, May 30. Team Roster and Code of Conduct forms can be downloaded below.
- To ensure player eligibility, complete and submit an official Team Roster (download Word document).
- To ensure sportsmanship, complete and submit a signed Code of Conduct (download Word document) by the head coach on behalf of entire team.
- To ensure each team is properly insured, submit your organiztion’s Certificate of Insurance (to be provided by you).
Email all items (roster, code of conduct, certificate of insurance) in one email with your team name and age group in the subject line to oyotournaments@gmail.com.
In 2024, we are remaining completely digital. We are not accepting hard copies onsite in order to ensure all documents are available electronically the day of the tournament. Thank you for your assistance, cooperation and understanding. Coaches also should bring copies of each player’s birth certificate, but they can retain those in the event any player’s eligibility is in question. These do not need to be submitted.
Lastly, these forms must be received from each participating team, including all Oaklandon teams. If not received, we will visit your field and stop the game in progress until missing forms are completed and submitted electronically — and we will be unable to add time back on the game clock during this stoppage in order to ensure the day remains on schedule.
